Adults
To register: complete and send the following forms with registration fees to state/province contacts:

Registration Form (1 page) pdf
Workshop Registration Form (1 page) pdf
Photo release (form at bottom of page - 1 page) pdf
Encouraged: Emergency Medical Information Form (place in a sealed envelope)

Teens (ages 15-18)
Teens, age 15 and older, are welcome to participate in the Forum. Youth under 18 must attend with an adult sponsor. Any youth under 15 must remain with an adult or guardian at all times. All teens under 18 must submit an Emergency Medical Information Form and a Youth Conduct Form signed by a parent or guardian and the name of the adult sponsor attending the Forum.
